• HEALTH SCREENINGS
    NYSED Health Exam Guidelines for Schools
     

     

    1) VISION SCREENING: 
         Vision screening is provided once a year to
         new students, pre-K, new kindergarteners
         (not screened in pre-K) and 1st graders.
     
     
    2) HEARING SCREENING: 
         Hearing screening is provided once a year to
         new students, pre-K, new kindergarteners
         (not screened in pre-K) and 1st graders.
                                                              
     
    3) PHYSICAL EXAM: 
         A physical exam given by your child's doctor needs to be submitted
         within 30 days from the first day they begin school for NEW STUDENTS,
         PRE-K, NEW KINDERGARTENERS and 1st GRADERS. If for some reason your
         child is unable to get a physical, our nurse practitioner or school
         physician will provide a very limited exam.
         A student's mental and physical health can impact their ability to
         participate fully in school. It is important for schools to have this
         information for students with medical concerns, fragile medical conditions,
         or social-emotional issues that require routine or emergent care by
         school personnel. A physical given one year prior to the first day of the
         school year is acceptable.
                                    
    PURPOSE OF THE PHYSICAL HEALTH EXAMINATION
                               
      1) To make an appropriate assessment of the student's current health status
      2) To determine whether the student is free from contagion and fit to attend and
           participate in all aspects of the school programs
      3) To indicate the need and extent to which the school program should be
           modified to benefit the student
      4) To communicate to the parent/guardian any findings which require further
           investigation and treatment
      5) To provide an opportunity to counsel the student and the parents/guardians
           concerning any health issues or conditions detected and/or securing
           appropriate care and support in the school environment
                                                    
                   *If you choose to decline any of the above health screenings,
                                you must send in a written and signed statement
                       to the Health Office stating you do not want your child given 
                                          a physical or a screening evaluation.


                                   The Health Office will notify you in writing,
                                            along with the screening results,
                              ONLY if there is an abnormal finding that would 
                                            require further medical diagnosis
