Falconer Central School Daily Student Survey Protocols
Parents are required to complete the daily survey that has been sent out to them for each day their child is scheduled to be in school:
- The District provided a tutorial that is on website showing how to set up a SNAP account and bookmark the site under: Daily Student COVID Screening
- This screening tool is offered for informational purposes to help you check for COVID-19 symptoms as outlined by the NYS Department of Health. If you answer “Yes” to any questions, please do not send your child to school. Please contact the school nurse and your health professional and share the answers to the questions below. You will need lab results or a statement from your health professional for your child to return to school. Without these things, your child will need to be home for 10 days. Those students with incomplete surveys will be called to the nurses’ office for screening.
o MS/HS and Fenner students (gr. 3-12) surveys are to be completed by 7:30 am daily
- School Messenger notification will be sent to parents that haven’t completed the survey at 8 am (Link attached to SNAP)
o Temple students (Pre-K-2) completed by 8:30 am daily
- School Messenger notification sent to parents that haven’t completed the survey at 9:30 am (Link attached to SNAP)

Falconer Central School District will be using "SchoolMessenger" as the Parent Notification System. You should all have received information regarding Opting-in to receive text messages. More information will be forthcoming regarding voice calls and email messaging availability. Please be sure to update all contact information on your students' "Student Information Form" and return to the appropriate school building. Thank you for your cooperation!
